East Bay Post High-CUE provides Transition Services for students in the Provo City School District

As students with disabilities prepare to leave High School at age 18 EBPH assists in the process of transitioning to adult life. We provide services in the areas of Post-Secondary Education (e.g. community/ local college, university, technical school), and Training (e.g. vocational, continuing adult education, on-the-job training), Employment (part -full time, paid, un-paid, internship, apprenticeship), Independent and Adult living (Social skills, Transportation, Community Access).

In addition to educational programs for school aged students 18-22, our school serves adults with disabilities ages 18 years and older. For over 30 years Provo School District has provided vocational services at CUE. An important aspect of the vocational, and educational programs for the "adult students" are the linen and laundry services, assembly work, along with custodial, food and other services for Provo district, it's patron's, along with dozens of businesses in the community.

The Mission of East Bay Post High-CUE is to provide instructional services to assist individuals to develop educational, personal, employment and independent living skills that will prepare them to work and reside as adults in their communities.

A key aspect of our educational services is to assist individuals and their families and care givers develop partnerships and linkages with community programs and resources that will serve them even after they leave the school district.

We facilitate work opportunities for all of our students. As part of our school we operate Central Utah Enterprises (CUE). CUE is a sheltered workshop and enclave program for adults with disabilities ages 18 and over. CUE does contract and production work both on and off site. CUE has been part of the Provo City School District for over 30 years.

Transition Coalition is a very good site. It includes training mudules with videos that make the learning more enjoyable and effective.

Here is an interesting report. It takes a current look at the Post School outcomes for students with disabilities in the US. The data they use is from 2005 through 2011 but even the older data is still relevant and useful. Transition report

As your student finishes High School it is natural to ask; "What's next?" EBPH can assist. Contact us at 801-375-0414 and press 4 for School Services or call 801-360-7135.

We will meet with you and your son/daughter in a Transition meeting. We can meet at school, home, or here at EBPH and discuss post high school plans and options. We work with all students 18 -22 years of age who are in the district and who have an IEP. Whether your son/daughter is planning on attending College, needing assistance preparing to work, developing independent living skills, or accessing community resources we can help. We can facilitate assistance through Accessibility Services for College (including UVU, BYU, MATC, Adult Ed. Continuing HS studies, etc.) work training (at EBPH through paid and volunteer training, Vocational Rehabilitation, Workforce Services, DSPD, etc.) Independent Living learning and Community access (Ability First, Recreational programs, etc.) We can help you and your student prepare for the next phase of life.

We can help you in understanding Age of Majority issues and Guardianship. We will refer you to those who can assist you through the process to gaining guardianship of your adult (18 year old) child.

During the school year East Bay Post High will provide information and resources that may be useful. It may be provided in one or more of the following ways:

Vocational Rehabilitation  VR's mission is to assist eligible individuals in obtaining employment and increasing their independence.

Utah State Office of Rehabilitation serves individuals through the Division of Rehabilitation Services (DRS), the Division of Services for the Blind and Visually Impaired (DSBVI), and the Division of Services for the Deaf and Hard of Hearing (DSDHH).

They assist individuals with disabilities who need assistance getting back to work, employers looking for qualified job applicants, or just want to learn more about rehabilitation in Utah go to this website.

Division of Services for People with Disabilities  DSPD promotes opportunities and provides support for persons with disabilities to lead self-determined lives. They oversee home and community-based services for people who have disabilities. Support includes community living, day services, supported employment services, and support for people with disabilities and their families.

Utah Parent Center- is the most comprehensive disability-related information and referral agency in the state of Utah.
